To mail or fax: Print out the form after entering your order information. Please note the extended totals and order total are calculated automatically once you enter the quantity (Qty) and cost per unit ($Unit).
To email: Saving the form to your computer will not retain the information you’ve typed in. To send as an email attachment, you must first print out the completed order form, then scan as a digital file (such as PDF).
